Mental Health Care Coordinator – Full Time

The Role:
As a Mental Health Care Coordinator, you’ll plan, manage, and implement care strategies for adults with mental illness. You’ll be a vital link between clients and the services they need, working closely with multidisciplinary teams and making regular community visits.

What We’re Looking For:

  • MSW preferred, but BSWs with 3+ years of mental health experience are encouraged to apply
  • Strong communication and problem-solving skills
  • A genuine passion for helping others
  • Must enjoy traveling within the Middletown area and surrounding areas
