Service Coordinator – Outreach to Homeless- (H2H) – $19.28/Hr

FT 40 Hr/wk Homeless to Housing (H2H) is an initiative designed to provide street outreach and engagement services, housing search and move-in, and linkages to community resources for people who are suffering from serious mental illness and/or substance abuse and are homeless or at imminent risk of becoming homeless in Eastern Connecticut. Supports are on-going once the individual is housed.
Worksite: Norwich
Hours: Mon- Fri, First Shift Flexible
Education and Experience:
- Preference given to those with lived experience
- Bachelor’s degree in human services or equivalent work experience in the provision of mental health preferred
- High School Diploma or equivalent required
- Valid CT Driver’s License, vehicle registration and insurance
- Good driving record
